To be an Elite caregiver you must have the following:

• Current provider first aid certification card
• Current provider CPR card
• Level two finger print clearance card
• Current Florida or state driver’s license
• Authorization to work in the U.S.A.
• Current auto liability insurance
• Reliable transportation
• Three references
• Minimum six months of care-giving experience
• Physical cleared completed by an MD,PA,OP,RN,ARNP or DO dated within six months
• HHA Certificate with HHA Curriculum
• CNA Must be Florida State Certificate
• HIV / OSHA Original Certificate
• Assisting with Self-Administration of Medications In-Service
• Domestic Violence In-Service
• Pass exam
